a condition that results from pressure on the median
nerve at the wrist is the most common upper extremity
compression syndrome.
Though the cause of this syndrome is usually unknown,
many factors may affect the pressure on the median
nerve in the carpal tunnel. These may include: fractures,
dislocations, swelling in the lining of the tendons
passing through the tunnel, and the actual size of the
individual’s carpal tunnel. Certain conditions such as
rheumatoid arthritis, thyroid problems, diabetes
mellitus, pregnancy and even extreme positions of the
wrist may affect the function of the median nerve.
CARPAL TUNNEL SYNDROME

SYMPTOMS of carpal tunnel syndrome commonly
include tingling or numbness in the thumb, index and
middle fingers. Initially, symptoms are often noted
from the wrist being held in certain positions during the
night. A burning sensation may radiate from the wrist
up the arm. Pain as well as numbness, and tingling can
be exacerbated with repetitive motions of the hand and
wrist. A decrease in sensation, strength, and ability to
manipulate objects with one’s hands may also occur.
EVALUATION includes a subjective history, followed
by an assessment of pain, sensation, range of motion,
swelling of the hand, and, upper extremity strength. A
review of the individual’s daily, vocational and
recreational activities is routinely performed. A plan of
care is subsequently developed based upon these
findings.
TREATMENT for non-surgical patients most often
includes the use of splints at night to maximize the space
in the carpal tunnel, and, hopefully alleviate symptoms.
Nerve gliding and other therapeutic exercises, as well as
manual techniques and modalities to decrease hand
swelling are often utilized. Patients are educated in
methods to modify their performance of activities in the
attempt to alleviate and prevent further symptoms. For
post-surgical patients, treatment often focuses on edema
reduction, scar management, and hand strengthening.
The GOAL of Occupational Therapy is to reduce and
prevent debilitating symptoms of the individuals upper
extremity, in order to improve the quality of one’s daily
activities and lifestyle.
